ATI HL2 Offer

I got an ATI Radeon 9600xt last year, came with the HL2 offer. Months ago I registered with ATI and mailed in all the stuff for the offer. I never got any confirmation e-mails about them recieving my stuff in the mail, and I assume I wasn't supposed to.

1)Should I have?
2)Is there a way to confirm whether they recieved what I sent and that I will in fact get my game?
3)Will I be contacted when the game goes gold or anytime before the game comes out?
4)Are they shipping the games to those getting a free copy before they are released in stores so that consumers won't have to wait as long?
5)How much for them to send me a CD-ROM of the game?
6)If I give them the money to send me a CD-ROM copy, will I get a boxed copy of the game, or will they be cheap and just send me a CD?


Just a bunch of questions I had about the offer. If anyone has an answer to any of these, post plz.

I think all you get is a working cd-key in which you can use to download it off steam. thats it.

You can get a CD copy of the game, that much I do know from the offer's site, I just don't know the cost or if it is the boxed copy or a CD.
