My scariest moment ever was the evil hand from the game blood that really gave me nightmares... The hand jumps at you and squeezes your head and your vision, and i never could get it off, SOOO creepy

hmm, scariest moment?
Would be Alien VS: Predator as a Marine. Walking down the hallway minding my business and then turn around and having 3 Aliens in my face. I mean really in my face. Like tunring 180 degree and all you see was a alien head and it's claw sliding across the screen. It made me almost shit myself. lucky me my awesomeness prevented it so all i did was piss myself. easier to clean

Hands down, fatal frame
walk down a hall, and its just a old creepy dusty hall, you look in your camera and all you see is people hanging, put it down, look again their faces are right in your face

Honestly, scariest for me was in F.E.A.R. where you come to a ladder to go down on a catwalk, hit the 'use' key, and as your character swivels around to reposition himself on the ladder properly you catch a glimpse of Alma on the platform you were just on.
I was so freaked out I slid all the way down the ladder, just to get greeted by Paxton Fettle at the bottom, where I promptly emptied an entire machine gun clip into thin air as he faded from view.
/darn you Alma, and your stinkin illusions!!!

FEAR is a walk in the park compared to AVP2.
AVP 2 is the only game that has every truly scared me. Every event triggered that releases Aliens still scares the Jesus out of me to this day.
FEAR is kind of creepy, but it's more the fear of the unknown than an actual being. Ghosts, yay. Nothing there right now, OK soldiers now, Wait more ghosts of nothing; I get it.
Biggest scare: AVP2 as Marine, Looking down at floor hatch which is getting torn up by Aliens, you can hear their clawing and hissing. Dead silence, followed by an Alien lungin out at you from the vent ABOVE the hatch.

I love horror games and had quite a lot of scares from titles such as Silent hill and Siren. The worst scare ever had to have come from the previous Alone in the datrk called the new nightmare and comes minutes after the game starts. I was sitting alone at night playing my PS1 and just bought the game and popped it in. After the plane crashed in the opening scene (if memory serves me right) I started walking around and went up to a gate and suddendly lightning flashed and displayed some shadowy figures. Completely unexpected and creeped me out.

Just to add something extra: The most scary thing in a game i've witnessed are the koreans in the 3rd coop level of Splinter Cell Chaos Theory. They are psycho and come running at you even if they dont see you, even scarier without nightvision, i almost peed my pants
